Do you have pharmacists based in this area?

Our pharmacists deliver the agreed service remotely from within the UK, working in your clinical system. The service does not require a clinician to be based locally.

Can you review care home residents without visiting them?

Where clinically appropriate, reviews can be delivered remotely with the patient involved and carers or care-home staff included as appropriate. The clinician uses the record and relevant clinical information, agrees a medicines plan and documents follow-up. Physical examination or further assessment is arranged through the agreed local pathway when needed.

Can you run diabetes clinics remotely?

Yes. Remote long-term condition clinics cover diabetes, hypertension, asthma and COPD, run to your protocols and recorded in your own system.

Can a whole PCN commission this together?

Yes, and care home work is one of the clearest cases for it, because homes often take residents from several practices in the same network.
