Clinical Pharmacist Support · Birmingham, West Midlands

Clinical Pharmacist Support for GP Practices in Birmingham

Large patient lists, real diversity, and medicines work sized to match. Birmingham practices run some of the largest and most diverse lists in the country, which means a steady volume of medicines work moving through every week. Our clinical pharmacists absorb it remotely, inside your own clinical system, as a managed service, not another hire.

The service is available to every practice and PCN in Birmingham. We hold no premises in the town and place nobody on site: because cover never depends on who can travel, the work gets picked up to the same standard every week.

Scale is the local problem in Birmingham

Birmingham’s practices are commissioned mainly by NHS Birmingham and Solihull ICB, though the city’s postcodes reach far enough that some practices on its edges are commissioned by neighbouring boards in the Black Country and Coventry and Warwickshire. Any provider working here has to be comfortable with more than one set of local priorities.

The defining local pressure is simply scale. Birmingham is the largest local authority in England, with a young and highly diverse population, and city practices commonly carry lists well above the national average. Volume of that kind changes which work slips: not the urgent appointments, which always get done, but the structured reviews, the monitoring recalls and the discharge actions that have no single deadline attached.

Language and health literacy add a second layer. A structured medication review that needs an interpreter takes longer and is easier to defer, which is exactly how a city practice builds a review backlog.

We are not a staffing agency and we do not place people into your building. You get a managed service, with supervision, cover, indemnity and clinical governance held by us.

What we take off your team

Each of these is a service in its own right. Take one, or take several together.

Structured medication reviews

Large, diverse lists build review backlogs faster than they can be worked. We take the backlog itself, not the easiest tenth of it.

Clinical medication reviews

Repeat prescribing

Repeat authorisation and acute triage at city volume, worked to your protocols so the queue stops growing while you look at it.

Repeat prescriptions

High risk drug monitoring

High risk drug recalls chased to completion, which is where large lists lose ground quietly and QOF notices later.

High risk drug monitoring

Discharge and clinical letters

Discharges from the city’s acute trusts reconciled and actioned, not filed to be dealt with when there is a gap.

Why diabetes drives the medicines workload in Birmingham

Most Birmingham practices record diabetes prevalence significantly above the England average. Diabetes is the condition that most reliably turns into medicines work: more patients on multiple medicines, and more structured reviews per thousand patients than an average list generates.

Diabetes9.98% higher than the England average of 7.89%
Hypertension12.81% lower than the England average of 15.23%
Chronic kidney disease3.69% lower than the England average of 4.64%

Figures are the median across GP practices in the area against the England value, from NHS Fingertips (National General Practice Profiles, QOF 2024/25). Only differences flagged as statistically significant are shown. Published NHS data about the area, not a statement about which practices we work with.

What the first three months usually look like

Indicative, and shaped by what you hand over. Nothing here needs a change to how your practice already works.

Weeks 1 to 2

Scope agreed against your actual list size, plus protocols and system access. No installation and no change to how your team works.

Weeks 3 to 6

The first workstream runs to a fixed weekly schedule. At Birmingham list sizes that is usually reviews or repeats.

Weeks 7 to 12

Reporting shows what was cleared and what it moved, which is normally when a second workstream gets added.

Excellent service from start to finish. Easy to reach, quick to reply, and they clearly know NHS workflows inside out. We couldn’t recommend the team more highly.

Frequently asked questions

Do you have pharmacists based in this area?

No, and we do not claim to. Our pharmacists work remotely from across the UK, inside your clinical system. Nothing about the service depends on anyone being nearby, which is exactly why cover stays consistent: it never turns on who can travel on a given day.

Our list is much larger than average. Does that change how this works?

It changes the scope, not the service. We size the workstream against your list; there is no fixed block of hours to outgrow. That sizing is the main reason large practices tend to get more out of this than small ones.

Some practices near us are commissioned by a different ICB. Does that matter?

Not to us. We work to the requirements your own commissioner is measuring. Where a network spans two boards we scope it explicitly, so one set of priorities is never assumed to cover both.

Can you support patients who need an interpreter?

We work within your practice’s existing interpreting arrangements rather than replacing them. What we change is that the review actually gets scheduled and worked, rather than deferred because it takes longer.

Can this be funded through ARRS?

In most cases, yes. The DES allows PCNs to fund roles like this through a service contract, not only through direct employment. We map the fit against your PCN’s funding allocation as part of scoping, so you know exactly where you stand before you commit to anything.
